Youth Projects Ltd is a large charity established in 1984 to support young people and families of the northwest suburbs and the central business district (CBD) of Melbourne.
We provide aspirational, life changing support through an integrated model in which multiple barriers are addressed with access to health care, counselling and drug safety, outreach services, employment, education and training.
About the Role
We are seeking the services of Community Health Nurses to join our casual pool of nurses.
The after-hours service operates Friday, Saturday and Sunday evenings from 7pm to 11pm, and provides assertive outreach to people in public spaces, crisis accommodation and rooming houses. The service addresses the immediate and primary care needs of patients, including health and social assessments, professional nursing care, counselling and active support, assistance with medication management, first aid and follow up
The successful candidate must be a Registered Nurse and have demonstrated triage nurse skills and experience managing injuries, other emergencies and providing coordinated care for people with multiple and complex healthcare needs. You will have an ability to arrange, implement and support referrals to specialist health and allied services. A history of working within the mental health and/or the alcohol and other drug sector is highly desirable.
